If you are a resident of Ontario living with a diagnosis of amyotrophic lateral sclerosis (ALS), primary lateral sclerosis (PLS), progressive muscular atrophy (PMA), progressive bulbar palsy (PBP), Kennedy’s Disease (KD) or certain hereditary spastic paraplegia (HSP) diagnoses, and you would like to access our supports and service, please complete the registration form using one of the options below:

After you fill out the PDF registration form, please email it to register@als.ca or fax it to 416-497-8545.

After your submission
Once we have received your form, you will receive an automatic notification confirming your submission (please check your junk mail if you don’t see it). A member of the ALS Canada Community Services team will contact you within two weeks.
Registration for other provinces
If you are not a resident of Ontario and would like to register for ALS support and services, please register with your ALS provincial society.
