The ALS Canada Equipment Program provides access to basic and essential assistive equipment through:
- Our loan equipment program, which includes new small bathroom items
- Funding assistance programs for certain leased and rented equipment

Please note that the ALS Canada Equipment Program is not a trial program. This means that we do not loan equipment for the purpose of testing what devices are best for you. Instead, ALS Canada will provide equipment on loan once an individual’s equipment needs have been assessed and preferably trialled by a healthcare professional. Delivery and pickup may cost anywhere between $100 and $400 and are covered by ALS Canada. This is made possible through generous donors in the community.
To apply for the Equipment Program, you must be registered with ALS Canada.
If you are registered, please have your Occupational Therapist, Physiotherapist, or other Registered Healthcare Professional complete the following forms:
Please note we are only accepting applications submitted through the new online forms. The online form is HIPPA compliant, and information is stored on a secure server based in Canada.
Once the application is successfully submitted, both the submitting healthcare professional and the client will receive a confirmation email from Social Solutions confirming receipt (please check your junk mail if not received).
